We collect information necessary to provide safe and appropriate medical care, including your contact details, Medicare information, medical history, medications, referrals, test results, clinical notes and billing information.

 

Your information may be collected directly from you or from healthcare providers involved in your care, including your GP, specialists, hospitals, pathology and imaging providers.

- provide and coordinate your healthcare;

- communicate with your treating healthcare professionals;

- manage appointments, referrals and test results;

- process Medicare, DVA and other accounts;

- maintain medical records; and

- meet our legal and professional obligations.

 

We may use secure medical and practice-management systems, including Gentu/Magentus, electronic messaging and My Health Record where applicable.

 

We take reasonable steps to protect your information from loss, misuse, unauthorised access or disclosure. Access to patient records is restricted to authorised practitioners and staff who require the information for legitimate clinical or administrative purposes.

 

We will not ordinarily disclose your health information to family members or other third parties without your consent unless disclosure is authorised or required by law.

You may request access to, or correction of, personal and health information we hold about you, subject to applicable legal requirements.

If a privacy or data breach occurs, we will investigate and respond in accordance with applicable Australian privacy and data-breach notification requirements.
